What is Tongkat Ali?

Tongkat Ali is the root extract of Eurycoma longifolia, a flowering plant native to Malaysia, Indonesia, Thailand, and Vietnam. The traditional use spans centuries — Southeast Asian medicine has used the root as a tonic for male vitality, recovery, and stamina long before modern endocrinology existed.

What changed in the last fifteen years is standardisation. Modern Tongkat extracts (LJ100, Physta®, Toniiq UHP) concentrate the active compound — eurycomanone — to between 1% and 10% of dry weight, with multiple human trials testing those exact extracts. "Tongkat Ali" without a standardisation number is mostly placebo; "Tongkat Ali, standardised to 2% eurycomanone, LJ100" is the supplement category that 10+ RCTs have actually measured.

It's the most-studied herbal lever for free testosterone outside of pharmaceutical TRT. That doesn't make it a TRT replacement (see "Who it's not for"), but it makes it the highest-evidence non-prescription move a man can make for natural T support.

How it works

Tongkat Ali's primary mechanism appears to be releasing testosterone from sex hormone-binding globulin (SHBG) — the protein that binds and inactivates ~60% of circulating testosterone. By disrupting that binding, eurycomanone increases the free-testosterone fraction available to androgen receptors. This shows up as a 10-20% rise in free T over 8-12 weeks in human trials, with the strongest effects in chronically stressed or aging men whose baseline free T is already compressed.

The secondary mechanism is cortisol suppression. Tongkat Ali appears to lower cortisol by 15-30% in chronically stressed subjects (Talbott 2013), which by itself raises the free-T-to-cortisol ratio — the metabolic environment that determines recovery, libido, and lean mass retention. This is why Tongkat consistently outperforms in stressed populations and shows weaker effects in young, low-stress men whose hormones are already optimised.

Third-order effects on sperm parameters, libido scores, grip strength, and lean-mass retention have been documented but are downstream of the two primary mechanisms above. The substance is not anabolic in the steroid sense — you do not gain 15 lbs of muscle on Tongkat Ali. You hold onto more of what you build, recover faster between sessions, and operate closer to your endocrine ceiling.

Does Tongkat Ali cause acne?

It can, in acne-prone users. Tongkat raises free testosterone by 10-20%, and more free androgens can mean more sebum — which in oily-skin users shows up as breakouts (face, back, shoulders) in the first weeks. Controlled trials rarely log acne as an adverse event, so it's not common, but user reports cluster around the 200-600 mg clinical doses. If it happens: drop to the low end of the dose range and run 8/2 cycling — it typically settles as your skin adapts. If you have a history of hormonal acne, that's a real reason to prefer Ashwagandha (cortisol-first, no direct androgen lift) instead.

Does Tongkat Ali actually raise testosterone, or is it placebo?

Should I cycle Tongkat Ali?

Can I stack Tongkat with Ashwagandha?
