Boswellia for Knee Pain: A Trial Found 61 % Less Pain in 8 Weeks. Which Extract Has Its Own Trial, and the 10 We Rank
In a placebo-controlled trial published in September 2026, 60 people with knee arthritis took 100 mg a day of one boswellia extract, OLNP-27. Their pain fell 61 % in 8 weeks, against 13 % on placebo. The maker of the extract paid for the study, and none of the 10 boswellia supplements we rank uses it. Here is which extracts have trials of their own, and the ranking.

A boswellia extract cut knee pain by 61 % in eight weeks in a new placebo-controlled trial, but that extract is not in the bottles most people buy. If you are here from our video, the two answers it promised come first: which extracts have trials of their own, and the boswellia supplements we rank.
Which boswellia extract has been tested for knee pain?
Three branded extracts have placebo-controlled knee trials, all run or paid for by their makers. OLNP-27 (100 mg a day) is the one from the September 2026 trial in Medicine. AprèsFlex, sold as Aflapin outside the US, was tested at 100 mg a day in trials published in 2010 and 2011. 5-Loxin was tested at 100 and 250 mg a day in 2008. A plain "boswellia 500 mg" capsule is not the same thing as any of them, because the active acids vary widely between extracts.
What are the best boswellia supplements?
We rank 10. None of them lists OLNP-27. Our #2, Life Extension, reads "5-LOX Inhibitor with AprèsFlex 100 mg" on its label, so it carries an extract with trials of its own. The ranking, by our score:
- #1 NOW Boswellia Extract, 9.1
- #2 Life Extension 5-LOX Inhibitor with AprèsFlex, 9.0
- #3 Terry Naturally Curamin, 8.5
- #4 Pure Encapsulations Boswellia, 8.5
- #5 Swanson Boswellia, 8.0
- #6 Nutricost Boswellia Extract, 8.0
- #7 Solgar Full Spectrum Boswellia, 7.7
- #8 Double Wood Boswellia, 7.5
- #9 Himalaya Boswellia (Shallaki), 7.4
- #10 Nature's Way Boswellia, 7.1
The short list below is the top three.
What did the 2026 trial find?
According to the trial report in Medicine, 60 adults aged 40 to 75 with mild to moderate knee osteoarthritis took 100 mg of OLNP-27 or a placebo once a day for 60 days. On a 0 to 100 pain scale, the extract group went from 68 to 26 and the placebo group from 68 to 60. The difference was already significant after 7 days. The overall knee score (WOMAC: pain, stiffness and daily function) fell 63 % against 17 %, and the inflammation marker hs-CRP dropped as well.
Who paid for the study?
The trial report says it was funded and supported by Olene Life Sciences, the company that makes OLNP-27. It was randomized, double-blind and placebo-controlled, which limits how much a sponsor can steer the result, but it was small and has not been repeated by an independent group. The AprèsFlex and 5-Loxin trials were also run by the maker of each extract.
How sure is this?
Moderately, for the short term. Several small placebo-controlled trials of different boswellia extracts point the same way for knee pain over one to three months, but each tested one specific extract, at one dose, in groups of 60 to 75 people. These trials measured symptoms such as pain, stiffness and daily function, not whether the joint itself changes. If you take prescription medication, ask your doctor or pharmacist before adding it.
How do you choose a boswellia supplement?
- Look for a named, tested extract on the label (AprèsFlex/Aflapin, 5-Loxin, OLNP-27) or a stated AKBA percentage.
- Match the dose to the trial: about 100 mg a day for these AKBA-enriched extracts.
- Give it at least one to two weeks; the trials saw the first differences after 5 to 7 days.
