GLP-1 Weight Loss Comes for Your Muscle Too — About a Quarter of What You Lose Is Lean Mass
Ozempic, Wegovy and Mounjaro work — but a big share of the weight you lose isn't fat. GLP-1 medications are genuinely effective, and the fat loss is real — this isn't an anti-Ozempic piece. But the scale hides something important: it can't tell you what you're losing. And on these drugs, a meaningful slice of it is muscle.
The number nobody mentions
In the SURMOUNT-1 DXA substudy of tirzepatide, of the total body weight participants lost, approximately 75% was fat mass and 25% was lean (muscle) mass — a ratio that held across most subgroups. Losing a quarter of your weight loss as muscle is why some people end up lighter but softer and weaker — 'skinny-fat' — with less strength and a slower resting metabolism, which also makes fat easier to regain if they stop the drug.
The fix isn't quitting the drug
You don't have to choose between the medication and your muscle. Two levers protect lean mass during any weight loss: enough protein and resistance training. In a meta-analysis of dieters, adding resistance training during a calorie deficit prevented roughly 93.5% of the lean-mass loss that dieting alone caused. Aim for about 1.6 grams of protein per kilogram of bodyweight a day, and lift two to three times a week.

The goal is simple: keep the appetite and fat-loss benefits of the GLP-1, and make sure the weight leaving your body is fat, not muscle. Prioritize protein at every meal — a quality whey or a well-formulated blend makes the target easy to hit — add basic resistance training, and you get the lean result people actually want, not just a smaller version of the same body composition.
